Building your Required Education File
Admission to the teacher education program does require the student to file certain forms with the Office of Teacher Certification before being admitted to the Teacher Education Program. Students who need to complete education file materials have the option of using the electronic forms linked to this page below. Blank forms may be printed for completion and submission to the department, or students may choose to complete the forms at their computer before printing them. Students who choose to mail the forms should send them to the Department of Education, 100 Gott Educational Center, Southwest Baptist University, 1600 University Avenue, Bolivar, MO 65613.
